  • If you are a brand new patient, please schedule for an Exam.

  • If you have had a gap in care of 1 year or more, please schedule for an Exam.

  • If you have had a gap in care of 3 months to 1 year, please schedule for a Re-exam.

  • If you have had a big health change, life event, or injury since your last appointment, please schedule for a Re-exam.

  • If you are seeing Dr. Molly regularly, please schedule for an Appointment.

  • If you are scheduling for a child 11 years old or younger, please schedule for a Child's Appointment.
